Subaxial cervical spine fractures in ankylosing spondylitis: management and outcomes from a 10-year population-based cohort.

This 10-year population-based cohort included 132 subaxial cervical spine fractures in 127 patients with ankylosing spondylitis (median age 70; 88% male), most from low-energy falls; 94% were AO type B or C and 20% had cervical spinal cord injury. Primary management was surgical in 67% and conservative in 33% (14% of conservatively managed patients required delayed surgery); revision surgery occurred in 9% and 90-day mortality was 16%, which was associated with older age, higher ASA class, and in-hospital pneumonia but not with cord injury or treatment modality.

Factor XI and XIa Inhibitors for Venous Thromboembolism Prophylaxis After Total Knee Arthroplasty: A Review From Mechanism to Phase III Trials.

This review examines targeting coagulation factor XI/FXIa for venous thromboembolism prophylaxis after total knee arthroplasty (TKA), summarizing biologic rationale and clinical trial data. Phase II programs using FXI antisense oligonucleotides, monoclonal antibodies, and small-molecule FXIa inhibitors demonstrated large reductions in venographic VTE with low bleeding profiles comparable to enoxaparin, and two pivotal Phase III programs are ongoing to define their role in contemporary post-TKA practice.

Retrospective Review of Outcomes of Conversion Total Hip Arthroplasty Following Failed Internal Fixation at Mean 3.1-Year Follow-Up.

In a retrospective series of 47 patients (mean age 66 years, mean follow-up 3.1 years) undergoing conversion hip arthroplasty after failed ORIF for hip fracture, Kaplan-Meier survivorship was 88.2% at one, three, and five years with an overall complication rate of 21.3% and a reoperation rate of 10.6% (most commonly infection). Harris Hip Scores improved significantly from a mean of 47.5 preoperatively to 83.3 at final follow-up, and all patients were ambulatory at follow-up with variable gait patterns.
