In this randomized trial of 1,184 patients with unprotected left main disease, there was no significant difference in 10-year all-cause mortality between PCI with contemporary drug-eluting stents and CABG (23% vs 25%; HR 0.93, 95% CI 0.74–1.18; p=0.56), with similar findings across SYNTAX score subgroups, supporting comparable long-term safety of PCI and CABG in patients eligible for either treatment.
